Hospice Care Includes:
- An interdisciplinary team of specially trained professionals to help care for each patient.
The team includes a physician, nurse, and social worker as well as a chaplain,
health aide, and trained volunteer as needed
- 24-hour on-call availability of a hospice medical professional
- Instructions to help those caring for the patient
- Assistance with personal hygiene care for the patient
- Respite services for caregivers, plus support and practical assistance from trained hospice volunteers
- Emotional and spiritual support for patients and their loved ones
- Bereavement support, including one on one support and bereavement support groups for surviving family members and caregivers after the death of a patient
- In-service programs for care-giving professionals
- Family and patient support groups in various locations
